Preface
This book has resulted from the combined editorial efforts of Heinz H. Bohmert and Ian T. Jackson who represent the discipline of plastic and reconstructive surgery from both sides of the Atlantic, and Henry P. Leis Jr, a general surgeon and specialist in breast cancer. The contributions of numerous thoughtfully selected, distinguished and recognized authorities in breast cancer treatment and breast reconstruction from all over the world are presented in an integrated fashion. Hopefully, it will provide an excellent source of information for the care of patients with malignant and premalig-nant disease of the breast, and special emphasis on conservation versus reconstruction. Multiple viewpoints are presented rather than a single biased opinion. This offers the reader an unusually complete and in depth coverage of the subject. In spite of this being based on the presentations at the Sixth Congress of the German Society of Senology with International Participation in Munich, this has emerged as a textbook rather than a proceedings.
The book starts with basic considerations relating to breast cancer. These include epidemiology and risk factors, prognosis and pathology. It discusses the concepts behind breast conservation treatment with emphasis on the changing concepts of breast tumor biology. Different authors present wide variations in the treatment of women with potentially curable breast cancers. Many different methods for reconstruction after mastectomy are discussed.
The pros and cons of immediate and delayed reconstructions are debated. Relatively simple reconstructive options such as implant placement and tissue expansion are compared with the more complex techniques of latissimus and rectus abdominis myocutaneous flaps. Reconstruction of the nipple/areolar complex is clearly and extensively described.
The very important area of the psychological effects of breast cancer and its treatment and the contributions made in this area by breast conservation and reconstruction are covered by several contributors. The management of local-regional recurrences, adjuvant chemoendocrine therapy and the management and meaning of premalignant lesions and in situ cancers are carefully discussed. For completeness, reduction mammoplasty and mastopexy have been presented. The vexed question of subcutaneous mastectomy has been given adequate airing.
It is hoped that this volume will provide not only a reference work, but also one which will be used in a practical manner in the management of this perplexing condition.
