Preface
Over the past 20 years significant advances have been made in the treatment of critically ill patients. To a major degree this is due to an improved understanding of the normal and pathological physiology of the lungs and the heart, and perhaps as important, to an appreciation of the interdependency of these two systems. The genesis of this book arose from a perceived need for a detailed exposition of the pathophysiological mechanisms of cardiopulmonary disease as seen in the intensive care unit as well as a logical approach to diagnosis and treatment based on these principles. The goal is therefore to provide the reader with insights into the disease process which transcend specific treatment regimens and thus should remain valid despite changes in present drug availability or technological advances in machines and monitors.
