Preface The present work was started at the Department of Paediatrics, The Copenhagen County Hospital in Gentofte. I thank the chief of the department, Dr. med. P. W. Brostrup for inspiration and for much practical help. The work continued during my appointment at the Department of Paediatrics of the University Hospital (Rigshospital). I thank Professor P.
