PrefaceNo field in medicine has experienced an explosion in knowledge in such a brief period as has the field of cardiology in the past few years. Enormous strides in our understanding of the pathogenesis of cardiovascular disease and the development of new drugs, techniques, and technologies for the diagnosis and treatment of heart disease challenge the clinician's ability to include these advances in the daily management of patients.In addition to established journals and books relevant to cardiologic practice, new journals, books, monographs, newspapers, and audio and video tapes appear almost weekly. It is a mammoth task to digest and monitor the flood of new information and impossible to instantaneously assess its value.The sixth edition of Clinical Cardiology attempts to present a comprehensive but not encyclopedic account of the newer concepts in diagnosis and therapy of cardiovascular disease in a lucid, balanced manner. New information is documented rather than presented in dogmatic fashion. The approach always starts with the foundation of etiology, history, and physical examinationin other words, the bedside evaluation of the patient that is the bedrock of clinical evaluation upon which the clinician bases decisions about which studies are necessary for diagnosis and for planning patient management. Details of the bedside evaluation as well as the technical procedures are described so that the reader can integrate the two. Both are essential for the proper modem management of the patient with cardiovascular disease.This book is written for medical students, medical house staff, cardiology trainees, internists, primary care physicians, and specialists in other fields who wish to "dip into" the book for specific information pertinent to their patients.A significant change in authorship has occurred in the sixth edition. Dr Melvin Cheit-lin, a coauthor of the fifth edition, has assumed responsibility as senior author. He has undertaken primary responsibility for updating the book in all matters pertaining to new advances in knowledge and changes in practice in the last three years. Dr Cheitlin is a clinician with 30 years of experience in the practice of cardiology. After 20 years in the United States Army Medical Department as Chief of Cardiology in the majority of the Army's medical centers, including Walter Reed Army Medical Center, he joined the University of California at San Francisco as Professor of Medicine and is currently Chief of the Cardiology Service at San Francisco General Hospital. Dr Sokolow, who was senior author of the first five editions, continues to be actively involved in the preparation of the book and has primary responsibility for the chapter on hypertension, which has now been totally rewritten. Dr Mcllroy has rewritten parts of the chapter on physiology.The considerable amount of new material has resulted in a modest increase in the size of the book, and new illustrations and references have replaced many of the older ones. Major new advances detailed in the book include extensive information on Doppler echocardiography, discussed throughout the book where appropriate, and new imaging techniques and agents for the diagnosis and treatment of coronary artery disease, valvular disease, endocarditis, pulmonary embolism, primary myocardial disease, and congenital heart disease.New invasive and noninvasive techniques are discussed in some detail, and their value is assessed.
