For many years as teachers, researchers, clinicians, consultants, and directors of coagulation laboratories, we have been asked many difficult but stimulating questions about hemostasis and thrombosis. These questions and the resulting dialogues have involved medical students, medical technologists, residents, and fellows oriented to hematology and oncology, academic and clinical colleagues and, very frequently, members of the practicing medical community. As a result of these conversations we realized that a need existed for concise information which could be easily understood and readily assimilated by the questioners, who were not always experts in thrombotic and bleeding disorders. Indeed, during the academic careers of the authors and particularly in the last several years, the field has exploded with new information, both basic and clinical, which has made it necessary for even the so-called "experts" to constantly review and struggle to place all of these new findings in perspective. Our efforts have been to try to capture this burst of knowledge and to provide the student and clinician a readily usable resource in the care of patients and their problems of thrombosis and hemostasis. In the second edition we have tried to retain the readability and concise nature of the first edition, despite the need to extensively revise most of the chapters and to add somé new ones. We had the very good fortune to recruit somé outstanding individuals to help in the review and revision of several of the chapters; we will be forever grateful to them for their precise and patient help and ad vice. This book, therefore, is the result of the collaborative efforts of the authors and their colleagues, who have attempted to combine their collective experiences to address important bleeding and clotting problems that affect all ages-from the newborn to the elderly.
