PREFACE_ Six years have passed since we pubhshed the last edition of this concise guide to the diagnosis and therapy of patients with cardiovascular disease. During this time, many important clinical trials and scientific observations have been reported in the medical literature. This new edition of the Manual of Cardiovascular Diagnosis and Therapy contains much information based on the results of these advances. The Manual will again be of considerable use to practicing physicians, house officers, and medical students in this challenging era of managed medical care. Dr. Gordon Ewy and I acknowledge the inspiration and assistance of our cardiology colleagues at the University of Arizona and throughout the world.
