Preface
Exciting developments have occurred In the field of coronary care since we first wrote this Manual. This Fourth Edition contains a considerable amount of new material. The original format has been maintained, however, with explanatory comments followed by specific protocol guidelines. The protocols are set in boldface type for easy identification. It is anticipated that initially readers will examine both the didactic and protocol sections of each chapter. Subsequently, they may need to refer only to the protocol section while planning the therapeutic strategy for a particular patient. Practicing physicians, house officers, nurses, and paramedical personnel have found the preceding editions of the Manual of Coronary Care to be most useful. It is our sincere hope that the Fourth Edition will be of even greater value.
As in the first three editions of the Manual, for each particular problem we present one particular approach that we have found effective. We recognize that other approaches may well be equally effective.
The authors acknowledge inspiration and assistance from the following: James E. Dalen, M.D., Joel M. Gore, M.D., and Charles I. Haffa-jee, M.D., of the University of Massachusetts Hospital; Jay N. Cohn, M.D., of the University of Minnesota School of Medicine; Eugene Braunwald, M.D., Lewis Dexter, M.D., Bernard Lown, M.D., and Thomas W. Smith, M.D., of the Brigham and Women's Hospital; and Robert A. O'Rourke, M.D., of the University of Texas Health Science Center, San Antonio. We are most grateful to the editorial staff of Little, Brown and Company for their encouragement and help.
