Foreword to the third edition The increasing number of specific questions on the systemic mycoses indicates that clinicians are becoming ever more aware of the problem of fungál infections of the internál organs in clinical practice. It is gratifying to note from such questions that isolation of fungi is now more critically evaluated than was formerly the case. It is to be hoped that the present guide will be consulted by specialists from fields other than internál medicine. It is in this spirit that I have added a short chapter on mycoses of the ear, nose and throat and the eye. Although we basically have at our disposal the same antifungal agents as when the second edition was published, important details (dosage, indications) in the chapter on therapy have been modified. Ali too often I have experienced that out of fear of the toxicity of amphotericin B the drug is given in insufficient dosage, with a fatal outcome.The proposal for a therapeutic procedure in status febrilis in patients with granulocytopenia, a common and still unsolved problem for clinicians, is new. The guide has been brought up to date by AIDS in that theT-cell immuné deficiency syndrome creates an ideál terrain for the occurrence of both mucosal and systemic mycoses. The dreaded cryptococcosis of the central nervous system, in particular, has shown an increase in these patients. To Dr Hans Scholer, that most experienced and always critical observer of the therapeutic scene, go my quite special thanks. His profound knowledge has been of valuable help to me. Sincere thanks, too, to Professor Hanne-Lene Mueller, whose work in the field of serology has been of great distinction. She read through the section on serology and provided new information.
