Preface
"New" cardiovascular diseases make their appearance known as physicians identify old diseases that were simply not recognized previously, or discover a cardiovascular disease that is truly new in that there is no past record of its existence. The "new" diseases may be poorly understood or may be the result of an obvious offending agent.
The designation of a cardiovascular disease as "new" is obviously arbitrary and can never be precise. The term "new" cardiovascular diseases, as used in this book, represents one clinician's view of diseases and disorders that are considered today but were not recognized earlier in his career. The conditions should be (and will be) discussed in modern editions of textbooks but were omitted in earlier editions of the same books.
I wish to thank the authors for their contributions to this book. I, as editor-in-chief, selected the topics and requested manuscripts from experts who know far more about the subjects than I do. Accordingly, I learned a great deal from the authors. I believe you, the reader, will also.
I thank Carol Miller, my editorial assistant, for her excellent help. I thank Gene Kearn, Vincent Polk, and Gita Bhattacharji at Igaku-Shoin who were devoted to the idea and production of the book.
Finally, as always, I thank my wife, Nelie, for constant support. No Nelie—no book.
