Preface
Although significant coronary artery disease remains the commonest cause of ischemic heart disease, myocardial ischemia may occur in the absence of severe coronary atherosclerosis. The aim of this monograph is to highlight various congenital and acquired conditions which produce myocardial ischemia other than coronary atherosclerosis.
Several chapters are devoted to congenital, familial and metabolic disorders which may produce myocardial ischemia. The effect of various causes of cardiac hypertrophy on myocardial blood flow is presented. Two chapters are included highlighting the pathophysiology and clinical aspects of epicardial coronary artery spasm. The possible role of changes in vasomotor tone of small vessels in precipi-tating ischemia is also discussed. Two chapters are devoted to coronary artery anomalies in the absence of major congenital heart diseases. One discusses the clinical aspects and the other is more descriptive with emphasis on the pathology. Other topics include coronary vasculitis, epicardial coronary artery aneurysms in the absence of severe atherosclerosis, coronary injuries secondary to trauma, and pathologie changes found in the myocardium in the absence of severe coronary disease.
The book is unique in that it presents in one volume a comprehensive review of causes of myocardial ischemia in the absence of significant coronary atherosclerosis.
