ForewordMedical management seems most secure when it is based on an actual image of the disease process. Certainly the demonstration of vessel pathology by an angiogram gives the clinician a firm footing for his treatment. A major goal of today's noninvasive methods of diagnosis is to portray the pathology without the difficulties and hazards of angiography. The images that noninvasive methods can generate may be adequate, yet not as clear as those produced by X-ray angiography. These methods may, nevertheless, compensate for this shortcoming by producing functional data of critical importance. Blood flow measurement, for example, may tell whether a vessel stenosis has or has not reached the level of a "significant stenosis."Some data given by noninvasive methods may rival in their specificity any of the accepted "specific" tests in medicine. Thus the demonstration, by the directional Doppler instrument, of a reversal of flow in the supraorbital artery is almost diagnostic of internal carotid occlusion or high grade stenosis. Again, the determination by the Doppler method or by impedance plethysmography, of a lack of free centripetal flow in a deep vein makes for a diagnosis of deep vein thrombosis with a high degree certainty. This volume has been produced by cHnicians for clinicians. Dr. Fehx and his collaborators are seasoned teachers and practitioners, all with considerable experience in the development of these methods and their use in the management of vascular disease. The reader can be assured they are highly qualified in assessing the current technology fully and well, and focusing clearly on the application of each method in the elucidation of specific vascular problems.Edward A. Edwards, M. D.Boston, Massachusetts
