elcome to the 32nci edition of The Washington Manual of Medical Therapeutics. The tradition of this publication began in 1943 as a handbook written by the Chief Residents intended for dissemination to the senior medical students and housestaff of Washington University's Department of Medicine. Wayland MacFarlane, MD, served as the first editor. In 1962, Róbert Packman, MD, dramatically altered the formát of the handbook, changing it from a full-page formát to a pocket-sized text. His goal at that time was to provide portable, accessible knowledge for a broad rangé of common diseases. Successive editions have grown in size and complexity, mirroring the practice of medicine. With the advent of portable technologies and internet resources, the Manual must adapt to meet the on-demand needs of physicians and students. With that in mind, we have altered the formát of the Manual from an annotated essay to an outlined structure encompassing etiology, clinical presentation, laboratory evaluation, and differential diagnosis, while maintaining a focus on therapy. We hope that this provides information on essential pathophysiology while maintaining an emphasis on emergency therapies vitai to the initial care of inpatients. Junior faculty and fellows author this text, stressing critical on-call, on-demand knowledge. Alongside the formát changes, we have painstakingly updated the content of the Manual to keep pace with changing technologies and therapeutics. As the best-selling medical text in the world, The Washington Manual of Medical Therapeutics has a tradition of excellence that we hope to uphold with this edition. We are proud to see it in the pockets of our housestaff and medical students, and on the shelves of our attendings (next to their copies of previous editions, of course). We are grateful for the assistance of the pharmacy staff at Barnes-Jewish Hospitál, particularly that of Scott Micek, whose help was essential in creating this Manual. We would alsó like to thank Lauren Aquino and the editorial staff at Lippincott Williams & Wilkins for their assistance. We have had the honor and pleasure of serving as Chief Residents of the ShatzStrauss, Karl-Flance, and Kipnis-Daughaday firms and the Wohl Clinic of the Department of Medicine at Washington University. To the house officers, we are moved by your fortitude and effort in what you do every day.
